29 CFR 1926.200(g)(2): Traffic control signs or devices used for protection of construction workers did not conform to Part VI of the Manual of Uniform Traffic Control Devices ("MUTCD"), 1988 Edition, Revision 3, September 3, 1993, FHWA-SA-94-027 or Part VI of the Manual on Uniform Traffic Control Devices, Millenium Edition, December 2000, FHWA. a) Employees were permitted to work in and near the traveled way of Main St. in Melrose Park, IL, without channelizing road users from the normal path to a new path as required by the Manual of Uniform Traffic Control Devices Millenium Edition Section 6C.05. b) Employees were permitted to work in and near the traveled way of Main St. in Melrose Park, IL, without the use of signage warning approaching road users of a one lane road as required by the Manual of Uniform Traffic Control Devices Millenium Edition Section 6F.20. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.201(a): The use of flaggers, signaling by flaggers or the warning garments worn by flaggers did not conform to Part IV of the manual on Uniform Traffic Control Devices ("MUTCD"), 1988 Edition, Revision 3, September 3 or Part VI of the Manual on Uniform Traffic Control Devices, Millenium Edition, December 2000, FHWA, which are incorporated by reference. a) An employee was permitted to direct traffic in the traveled way of Main St. in Melrose Park, IL, and did not confrom to Part VI of the Manual of the Uniform Traffic Control Device Section 6E.4. The employee was not provided a sign paddle or flag to safetly transfer vehicular traffic. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a) Employees working in an excavation approximately 5.8 feet in depth were exposed to cave-in hazards. Norridge Sewer & Water Construction, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1) which was contained in OSHA violation number 311163174, citation 1, item 3, issued on August 2, 2007 and became final order on or about August 24, 2007 with respect to a workplace located at 1522 Douglas, Arlington Heights, IL 60005, and OSHA violation number 312593601, citation 1, item 1, issued on April 23, 2009 and became final order on or about May 20, 2009 with respect to a workplace located at 111 Chicago Ridge Mall Drive, Chicago Ridge, IL 60415. No abatement certification or documentation is required for this item.
