29 CFR 1910.269(d)(2)(iii): Procedures were not developed, documented, and used for the control of potentially hazardous energy covered by paragraph (d) of this section.    a) FirstEnergy Generation Corp. - Oregon, OH: On or about May 20, 2011, the employer did not assure that energy control procedures were utilized to control hazardous energy when the 3-1 air heater bearing oil temperature switch was serviced.  Employees were exposed to electrical hazards when servicing 120VAC equipment.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.269(d)(6): The established procedure for the application of energy control (the lockout or tagout procedures) was not done in sequence and did not include the elements and actions of 29 CFR 1910.147(d)(6)(i) through (d)(6)(vii):    a) FirstEnergy Generation Corp. - Oregon, OH: On or about May 20, 2011, an employee(s) was exposed to electrical hazards in that the 3-1 air heater bearing oil temperature switch was not locked out prior to servicing the switch.  Prior to accessing the exposed live parts of the temperature switch, the employer did not assure that energy control application steps were performed in that the equipment was not shut down, the energy isolating device(s) was not operated in a manner as to isolate the machine from energy sources, a lockout device(s) was not affixed to the energy isolating device(s), and verification of isolation was not conducted.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
