29 CFR 1910.147(c)(1): \\r\\n\\r\\nEnergy control program. The employer shall establish a program consisting of energy controlprocedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine orequipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall beisolated from the energy source and rendered inoperative.\\r\\n\\r\\na. R & S Technologies, Inc. located in Bellevue, Ohio.  On or about August 22, 2011, the employer did not ensure employees changing out mandrel on the Brother TC-227 CNC were locking out the equipment prior to working on the equipment.  Employees were exposed to the unexpected rotation of the spindle and movement of the tailstock.\\r\\n\\r\\nIn accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. The employer shall record the annual maintenance date and retain this record for one year after the last entry or the life of the shell, whichever is less. The record shall be available to the Assistant Secretary upon request.     a. One or more methods of machine guarding shall be provided to protect the operatorand other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips andsparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.\\\\r\\\\n\\\\r\\\\na.  R & S Technologies, Inc. located in Bellevue, Ohio.  On or about March 16, 2011, the employer did not ensure employees placing their hands near the tailstock and the tooling chuck, to place parts onto the mandrel, were protected from the horizontal movement of the tailstock and the rotation of the tooling chuck on the Brother TC-227 CNC machine.  An employee's fingers were pinched between the part and the tailstock while the employee held a part on the mandrel. \\\\r\\\\n\\\\r\\\\nIn accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. The guard shall be such that it does not offer an accident hazard initself.    a)  R & S Technologies, Inc. located in Bellevue, Ohio.  On or about August 8, 2011, the employer did not ensure the front automatic door on the Brother TC-227 CNC machine, which acts as a guard when the equipment is in operation, did not cause a hazard.  As the door closes, a pinch point is created between the door and the frame of the machine.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
