29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employer: On or about July 19, 2011, at the above addressed jobsite, safety inspections had not been conducted by persons capable of identifying and correcting hazards, such as inadequate anchorage points for the fall arrest system lifeline.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about July 19, 2011, at the above addressed jobsite, employees were exposed to falls while working near the roof edge. The employees were out fitted with body harnesses/lanyards, which were not in use, however there was only a single lifeline available and that lifeline was looped around an exhaust fan roof support frame made from 2"x4"s which were screwed into the roof deck. The employees were exposed to fall hazards. MIDWEST WRECKING COMPANY WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313933137, CITATION NUMBER 1, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON APRIL 1, 2010, WITH RESPECT TO A WORKPLACE LOCATED AT 846 W. RANDOLPH STREET IN CHICAGO, IL.
