On or about July 29, 2011, at the above addressed jobsite, a safety and health program, which would address those hazards associated with the employees' work environment and the company's procedures to eliminate those hazards had not been developed and maintained.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards:    a. On or about July 29, 2011, at the above addressed jobsite, an employee training program addressing fall hazards and the company's procedures to be followed to protect the employees from exposure to those hazards had not been developed and maintained.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet(1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, personal fall arrest system or an alternative fall protection system:    a. On or about July 29, 2011, at the above addressed jobsite, employees were exposed to falls of approximately 19 feet due to the lack of fall protection (i.e. body harnesses & lanyards attached to lifelines) on the 12/12 pitched roof.
