29 CFR 1926.451(g)(1)(i): Each employee on a boatswains\' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:      (a)  An employee of BNW Installation, Inc. was not protected from falling while conducting roofing work on a ladderjack scaffold.  This exposed the employee to a fall hazard of approximately 11 feet.    THE RBD CONSTRUCTION DBA CEASAR AVIA WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1926.451(g)91)(i), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 315119040, CITATION NUMBER 2, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON MARCH 28, 2011, WITH RESPECT TO A WORKPLACE LOCATED AT 8282 64TH COURT, PLEASANT PRAIRIE, WISCONSIN.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.  (a)  The employer failed to ensure that each employee conducting roofing work at 4314 21st Avenue, Kenosha, Wisconsin, were protected from falling to the lower level by the use of a fall protection system.  This exposed employees to a fall hazard of approximately 12 feet.    THE RBD CONSTRUCTION DBA CEASAR AVIA WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1926.501(b)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 315119040, CITATION NUMBER 2, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON MARCH 28, 2011, WITH RESPECT TO A WORKPLACE LOCATED AT 8282 64TH COURT, PLEASANT PRAIRIE, WISCONSIN.    THE RBD CONSTRUCTION DBA CEASAR AVIA WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1926.501(b)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313135360, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS ISSUED ON SEPTEMBER 27TH, 2010, WITH RESPECT TO A WORKPLACE LOCATED AT 1109 GREIG TRAIL, STOUGHTON, WISCONSIN.
