DBA Tiny's Granite Works at 4692 Glencoe St. in Denver, CO: The Hyster and Clark forklifts were equipped with front-end attachments other than factory installed attachments and the forklifts were not marked to identify the attachments and show the approximate weight of the truck and attachment combination.  The Hyster forklift was fitted with a Hercules attachment and the Clark forklift was used with a T&S attachement. This condition exposed employees to a forklift hazard. One respirator was observed in the shop hanging from a hook with dust accumulation on the facepiece. A second respirator appeared to have grease or another dark substance adhered to the facepiece.
