29 CFR 1926.451(c)(2)(i): Footings were not level, sound, rigid and capable of supporting the loaded scaffold without settling or displacement: \r\n(a) Mountain High Masonry Inc. at 3992 Havana St, Denver, CO 80239: On and before August 6, 2011, the exposing employer did not ensure that baseplates on the fabricated frame scaffolding along the west side of the building were level, sound, rigid and capable of supporting the loaded scaffold without settling or displacement, in that some baseplates were extended over and not secured to the wood planks beneath the baseplates. This condition exposed employees to the hazard of scaffold shifting and/or collapse.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:\r\n(a) Mountain High Masonry Inc. at 3992 Havana St, Denver, CO 80239: On and before August 6, 2011, the exposing employer did not ensure employees working on a scaffold were protected from falls via the use of guardrails along all open sides and ends of the scaffold. The scaffold used on the west side of the building lacked mid-rails and end-rails. Employees worked on the 2nd level working platform of this scaffold. This condition exposed employees to a fall of approximate 14 feet.\r\nMountain High Masonry Inc. was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1926. 451(g)(4)(i) which was contained in OSHA Inspection Number 313717159, Citation 1, Item 3a, issued on February 9, 2010, and became Final Order on April 19, 2010, with respect to a workplace located at 9364 S Jordan Rd., Parker, CO 80134.\r\n
