29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. On or about June 03, 2011, at a jobsite located at 16130 Oak Park Avenue, Tinley Park, Illinois - employees working beneath employees performing masonry activities from a tubular welded framed scaffold were not protected by the use of a hard hats, thereby exposing the employees to struck by hazards.

29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. On or about June 03, 2011, at a jobsite located at 16130 Oak Park Avenue, Tinley Park, Illinois - employees were performing masonry activities from a tubular welded framed scaffold that was not equipped with end rails, thereby exposing the employees to falls of approximately twenty two (22) feet.
