29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall beguarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards,shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operatingcycle. a. In the maintenance area, the point of operation on the Birmingham Lathe, model YCL1660, serial number 810805, was not adequately guarded in that employees were allowed to wear gloves while operating the machine and the guard on the machine did not prevent contact with the chuck. On or about August 8, 2011 an employee operating the Birmingham Lathe, model YCL1660, serial number 810805 was injured when his right hand was caught in the lathe chuck resulting in an amputation of the tip of the right index and middle fingers.
