29 CFR 1910.134(h)(1)(i): Respirators issued for the exclusive use of an employee were not cleaned and disinfected as often as necessary to be maintained in a sanitary condition: a) On or about 10 August 2011, the employer did not ensure that supplied air respirator(s) used by employee(s) in the sandblasting booths were cleaned and disinfected as often as necessary to be maintained in a sanitary condition. Upon inspection of the supplied air respirator(s), the inside parts of the respirator(s) were found dirty and contaminated with metals including but not limited to Copper, Manganese, Zinc and Iron and visible dust containing Crystalline Silica. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve: a) On or about 10 August 2011, the employer did not ensure that supplied air respirator(s) used by employee(s) in the sandblasting booths were stored in a manner that would protect them from contamination and dust. The supplied air respirator(s) used during blasting operations were stored inside the blast room(s), exposed to contaminated dust containing hazardous chemicals including, but not limited to: Crystalline Silica, Copper, Manganese, Zinc and Iron. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.1025(h)(2)(i): Floors and/or other surfaces where lead accumulates were cleaned by the use of compressed air: a) On or about 10 August 2011, the employer did not ensure that employee(s) did not clean the supplied air respirator(s) using compressed air at the end of their work shift. The blasting helmet(s) were contaminated on the outside with hazardous chemicals including but not limited Crystalline Silica, Copper, Manganese, Zinc and Iron Oxide dust. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.1025(l)(1)(ii): The employer did not train each employee who is subject to exposure to lead at or above the action level, or for whom the possibility of skin or eye irritation exists, in accordance with the requirements of 29 CFR 1910.1025: a) On or about 12 August 2011, the employer did not institute a training program for those employee(s) for whom the possibility of skin irritation existed. The employee(s) did not know the health hazards associated with lead exposure. The presence of lead on the hands (15.6 �g and 125.3 �g, respectively) of two employees returning from sand blasting in Blast Rooms # 1 and # 2 was detected following wipe sampling at the end of their workshift on 12 August 2011. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
