29 CFR 1926.251(e)(8)(iii): Synthetic web slings were not immediately removed from service when the condition of snags, punctures, tears or cuts were present. \r\n\r\n(a) All County Crane Services, Inc. on or about August 10, 2011, at the address of 11015 SR 250 N Milan, OH: The employer failed to ensure synthetic web slings used to hoist an approximately 1600 lb HVAC unit were removed from service when several snags and cuts were present in two of the four legs. When using the damaged 20 ft long nylon slings, employees were exposed to struck-by injuries caused by fallen equipment/material.

29 CFR 1926.1404(g)(2): Outriggers were not set to remove the equipment weight from the wheels All County Crane Services, Inc. on or about August 10, 2011, at the address of 11015 SR 250 N Milan, OH: The employer failed to ensure the outriggers of the JLG model #170JBT mobile crane were fully deployed ensuring the cranes weight was removed from its tires. Weight placed on the pneumatic tire decrease the crane stability, exposing employees working on or around the crane to overturning hazards.
