29 CFR 1910.178(l)(4)(ii)(B): \r\n\r\nRefresher training in relevant topics was not provided to the operator when the operator had been involved in an accident or near-miss incident\r\n\r\na. At the site, on or about March 6, 2011 an incident occurred when a powered industrial vehicle was backed up by an operator and bumped another employee. The operator was not retrained before being allowed to operate the vehicle again.
