At the dock area where employees were changing a overhead garage door which had been damaged, the employer did not develop and document procedures to safely ensure the energy was at zero and the proper procedures for replacing the overhead garage door thereby exposing employees to a struck-by hazard.

At the shipping/reciving dock where two employees were changing an overhead door panel, the employees was not provided information and training on the proper procedures for removing the energy of the coil spring before removing the door panel thereby exposing employees to a struck-by hazard.
