The employer failed to record an amputation that occurred in the High Density department (HD) on October 20th of 2009 while operating the Air Mite Press End Disk Applicator. This case was recorded by the employer as a fractured finger the Logs of Work-Related Injuries and Illnesses OSHA 300. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
