29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees.  The training shall include the following:    a.  An employee designated by the Company as an "authorized employee" had not received training upon hire or evaluated by the Company as to their knowledge and skills required for the safe application, usage, and removal of energy control devices.      b.  Employees who grease the chuck on an Okuma Lathe had not received training on the knowledge and skills required for the safe application, usage and removal of energy controls.     c.  Employees who clean the base plate on Muratec MD 120 SN 4524 had not received training on the knowledge and skills required for the safe application, usage and removal of energy controls.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees.    a.  In the Production Area, VVT, the Muratec MD 120, SN 4524  electrical and pneumatic energy sources were not locked out while employee(s) open the plexiglass door at the beginning of the shift to clean the base plate in which a part was placed on by a robotic arm.    b.  In the Production Area the electrical energy source was not locked out while employee(s) greased the chuck on the Okuma Lathe.    c. In the Production Area the pneumatic energy source was not locked out while employee(s) replaced the sensors on the tool changer of the Kitamura, My-Center 2, SN 11185.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards,shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    In the Production Area the point of operation on the Leak Tester Machine exposed an employee to a crushing injury due to the presence sensing devices being located too close to the edge of the downward movement of the ram of the machine, 4", versus 5.5" as the calculated safe distance.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) shall be visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).  Cord and plug connected equipment and flexible cord sets (extension cords) which remain connected once they are put in place and are not exposed to damage need not be visually inspected until they are relocated.    In the Custom Machining Area a floor fan (no manufacturer or model identified on fan) being used by employees during a period when the air conditioning was not working had a plug missing its ground pin and a section of internal insulated wiring was exposed due to the outer insulation being pulled away from the fitting.
