29 CFR 1910.23(a)(3)(i): All hatchways and chute floor openings were not guarded by a standard railing:    a) Kitchen- The employer failed to ensure employees were provided with a standard railing while in the area around the Kitchen hatchway.  This open hatchway exposed the employees to fall hazards on three of the sides.    Tolgano Inc. dba Wolcott's was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1910.23(a)(3)(i), which was contained in OSHA Inspections Number 315419614, Citation 1, Item 1, issued on April 15, 2011, with respect to the workplace at 1834 W. Montrose, Chicago, Illinois.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
