29 CFR 1904.32(a)(2): Create an annual summary of injuries and illnesses recorded on the OSHA 300 Log;    a) At the job site on August 10, 2011 and at times prior thereto, the employer did not produce the summary upon request for calendar year 2007.    b)  At the job site on August 10, 2011 and at times prior thereto, the employer did not produce the summary upon request for calendar year 2008.    Note: Abatement certification IS required for these items.

29 CFR 1910.95(d)(1): When information indicates that any employee's exposure may equal or exceed an 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program.     (a)     Log Deck: On or about August 10, 2011, and at times prior thereto, the employer did not conduct monitoring to assess noise exposures for an employee using a chainsaw to manually delimb trees.     (b)    Log Deck: On or about August 10, 2011, and at times prior thereto, the employer did not conduct monitoring to assess noise exposures for an employee operating a Link-Belt Delimber 3400.     (c)     On or about August 10, 2011, and at times prior thereto, the employer did not conduct monitoring to assess noise exposures for an employee using a chainsaw to manually fell trees.     Abatement Note: Where employees noise exposures equal or exceed 85 decibels over an 8-hour time-weighted average, the employer must administer a continuing, effective hearing conservation program, as described in paragraphs (c) through (o) of this section.  This must include but is not limited to:     1.       additional monitoring where exposure levels may increase (new equipment, process, etc);   2.       baseline and annual audiometric testing;   3.       providing suitable hearing protectors from which employees can choose; and  4.       training on the hazards of noise and the hearing conservation program, to be repeated annually.       Abatement Note: Abatement certification IS required for this item.
