Barre Rd. in Archbold, OH:  The employer failed to assure employees that were exposed to electrical hazards while performing work on or near exposed energized parts were trained in electrical safety-related work practices. Employees who were not trained were exposed to electrical hazard while replacing overhead lighting fixtures.\r\n\r\nIn accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    Thomann Electrical Service, Ltd. on or about August 16, 2011, at the worksite located at 801 W. Barre Rd. in Archbold, OH:  The employer failed to assure that employees testing and verifying the absence of electrical energy while replacing lighting fixtures were provided with, and used, protective equipment for the hands, face, head, and body.  Employees were exposed to electrical shock, arch flash/blast, and/or electrocution when performing verification tests to the 480 volt lighting circuits.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
