29 CFR 1926.403(b)(1): The employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:\r\n\r\n(a) On or about August 5, 2011, GA Roofing & Construction, as controlling employer, did not ensure that employees of Elmer Ulises Miranda were protected from shock, burn and fire hazards in that the extension cord used to supply power to the leaf blower that the employees were using on the roof at 6039 W. Mexico Ave. Lakewood, Colorado, had a cut in the insulation.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan: (a) On or about August 5, 2011, GA Roofing & Construction, as controlling employer, did not ensure that employees of Elmer Ulises Miranda were protected from fall hazards of greater than eight feet while conducting reroofing activities on a house at 6093 W. Mexico Ave. in Lakewood, Colorado.
