Mexico Ave. Lakewood, Colorado, had a cut in the insulation.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders which would enable each employee to recognize hazards related to ladders and train each employee in the procedures to be followed to minimize these hazards. (a) On or about August 5, 2011, the employer did not ensure that employees were able to recognize and mitigate the hazards of ladders while using a ladder to access a roof at 6039 W. Mexico Ave., in Lakewood, Colorado: 1) the top of the ladder was not extended at least three feet above the landing surface. 2) the feet were placed on a concrete surface but one foot was up and the other foot was down.
