29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    a) Olmsted Falls Erecting Co. on or about August 16, 2011, at the address of 151 Innovation Dr. Elyria, OH: The employer did not ensure an employee was wearing appropriate eye protection such as, safety glasses while using a Bosch angle grinder (model #1375a) to prep metal for welding. The employee using this tool without eye protection was exposed to potential eye injuries from flying particles.

29 CFR 1926.300(b)(1): Power operated tools when designed to accommodate guards, were not equipped with such guards when in use.    a) Olmsted Falls Erecting Co. on or about August 16, 2011, at the address of 151 Innovation Dr. Elyria, OH:  The employer did not ensure a Bosch angle grinder (model #1375A) was equipped with the appropriate factory provided guard while in use.  Employees were using the unguarded tool to grind structural steel.  The employees were exposed to potential contact of the blade during grinding operations.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems.    Olmsted Falls Erecting Co. on or about August 16, 2011, at the address of 151 Innovation Dr. Elyria, OH: The employer did not ensure employees were protected from a 42 ft fall to the ground below during steel erection work. No acceptable form of fall protection was in place.    Olmsted Falls Erecting Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.760(a)(1), which was contained in OSHA inspection number 311497275, citation number 1, item number 1 and was affirmed as a final order on May 1, 2008, with respect to a workplace located at 236 Crocker Park Blvd. Westlake, OH.
