29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.      On August 17, 2011 employees were not provided fall protection while walking/ working on the roof of a commercial building, exposing them to a fall hazard of 24.5 feet.    Abatement documentation is required of this item in accordance with 29 CFR 1903.

29 CFR 1926.1053(b)(1): \r\n\r\nWhere portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:\r\n\r\n\r\nOn August 17, 2011 employees were using an extension ladder that did not extend three feet past the upper landing, exposing them to the hazards associated with a fall.\r\n\r\n\r\nAbatement certification is required of this item in accordance with 29 CFR 1903.\r\n\r\n\r\n
