29 CFR 1910.22(c):     Covers and/or guardrails were not provided to protect personnel from the hazards of open pits, tanks, vats, ditches, etc.    American Pennzoil, located at 2919 West Elm St., Lima, OH: On or about August 17, 2011, the employer failed to provide covers and/or guardrails for open pits in the shop when pits were not in use. Employees were exposed to fall hazards of approximately 6 feet from the shop floor to the work platforms located in the pits.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. A.	American Pennzoil, located at 2919 West Elm St., Lima, OH: On or about August 17, 2011, the employer did not ensure that employees, performing routine vehicle maintenance such as, but not limited to, oil changes, wore eye protection such as safety glasses. Employees were exposed to eye injury hazards from flying particles while working in pits underneath vehicles.    B.	The employer did not ensure that employees wore approved safety goggles or a face shield when handling the hazardous products such as, but not limited to, Armor All Professional Orange Body Shield (Isopropyl Alcohol), ZEP vehicle wash XT-1398 (corrosive), Challenger Presoak low pH multifunctional acid detergent EXT118 (Ammonium Bifluroide), Black Night highly concentrated alkaline detergent EXT432 (Sodium Hydroxide), and ZEP Z-Maxx drying agent X-3306 (Ethylene Glycol Monobutyl Ether). Employees are required to refill and mix these products in order to operate the car wash. Employees were exposed to eye injury hazards from the splash of liquid chemicals contained in the hazardous products when employees were refilling and mixing the products.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
