In the Deburring Station Room in Building 1, the 110 V AC power cord plug was not unplugged prior to removing the end guard on the B&D abrasive wheel grinder. b. In the Saw Department in Building 3 1/2, on or about June 19, 2011 and thereafter, the 220 V AC power cord plug was not unplugged prior to changing the saw blade on the Scotchman Industries, CPO 350 NF Cold Saw, Serial Number B1759NF1007.

In the Deburring Station Room in Building 1, the 110 Volt AC power cord plug was not unplugged prior to removing the end guard on the B&D pedestal abrasive wheel grinder, #6018 to replace the carbide wheel. b. In the Saw Department in Building 3 1/2, on or about June 19, 2011 and thereafter, the 220 Volt AC power cord plug on the Scotchman Industries, CPO 350 NF Cold Saw, Serial Number B1759NF1007, was not unplugged prior to changing the saw blade.

Employees using a voltmeter on an energized 480 Volt AC main electrical circuit had not received training on what personal protective equipment was to be worn to protect himself against electric shock and arc flash.

On or about August 15, 2011, on Building 2 roof, employee(s) performing voltage testing using a voltmeter on an energized 480 Volt AC main disconnect electrical circuit for a HVAC unit, Bryant, Model 579DEW048120AABG, were not wearing the personal protective equipment consisting of FR clothing to protect themselves against arc flash.
