29 CFR 1910.67(c)(2)(v):     A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift.    (a)  An employee conducting maintenance operations  from a JLG 660 aerial lift was not protected from a fall/thrown hazard by using a safety harness and lanyard attached to the lift.
