29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    On or about August 18, 2011, at the facility, employees required to access and work atop of tankers/trailers/railcars during chemical loading and off-loading operations were exposed to fall hazards from a height of approximately 10 to 12 feet above ground.  Protective equipment, such as a body harness and lanyard, was not used to protect against injury and impairment in the event of a fall.     Loading and off-loading operations were conducted contiguous to loading racks/platforms where the installation of fall protection was feasible.
