29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Terminating Department - Employees were exposed to amputation hazards while working on terminating machines due inadequate guarding at the point of operation. The employee could reach around the front of the guard into the point of operation. Machine guarding includes all terminating and braiding machines. (b) Terminating Department - Employees were exposed to amputation hazards due to the foot pedals not properly guarded. The accidental activation of the machine and improper guarding of the point of operation could cause amputation. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(c).

SK Express, Inc - On or about August 18, 2011, the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar years 2008 and 2011. - On or about May 9, 2008, an employee fainted in the layout department. - On or about July 22, 2008, an employee fainted shortly after arriving to work. - On or about March 29, 2011, an employee obtained a fracture and laceration to the left index finger from terminating machine. Abatement Certification and Documentation is required of this item in accordance with the requirements of 29 CFR 1903.13(d).
