29 CFR 1926.501(b)(11):  Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) R & R Remodeling:  On or about August 17, 2011, at address 401 W. Airport Hwy., Swanton, Ohio, the employer did not ensure that employees were protected from falls while removing old roof materials and applying new roofing materials to a steep sloped roof (5:12) on a commercial type structure.  Employees were exposed to falls of approximately 9 ft. to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:    a) R & R Remodeling:  On or about August 17, 2011, at address 401 W. Airport Hwy., Swanton, Ohio, the employer did not ensure that employees had been trained in the appropriate use of fall protection, such as but not limited to the use of personal fall arrest systems or alternative measures while performing various roofing operations.  Employees were exposed to falls of approximately 9 feet to the ground below, which could result in temporary or permanent disability or death.
