29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: a) Saylor Tree Service & Stump Removal, LLC, Napoleon, Ohio: On or about August 17, 2011, at address 1032 Scott St., Napoleon, Ohio, the employer did not ensure that each employee performing tree service from an elevated vehicle mounted platform wore a personal fall arrest system. An employee performing work in this condition was exposed to falls of approximately 15-20 feet while engaged in this activity.

Employees were exposed to potential impact injuries caused by falling branches and/or equipment.
