29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.  a) Located along various areas of the 6 in 12 sloped residential roof structure employees performed or were observed performing roof felt insulation roofing work and sheeting without conventional or compliant alternative fall protection devices or methods and were exposed to fall potentials in excess of 10'.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Located along various areas of the residential roof structure employees performing of observed performing roof felting and roof sheeting operations without conventional fall protection or following a fall protection plan adjacent to unprotected sides or edges were not trained or knowledgeable of procedures or regulations to be followed to minimize those hazards.
