Inc. at 9575 East 3rd Place, Denver, CO 80230: On or before August 20, 2011, the employer did not ensure that employees wore safety glasses when using or working in the vicinity of pneumatic nail guns. Condition exposed employees to the hazard of projectiles to the eyes. Abatement note: Abatement certification is required for this item (see enclosed ,"Sample Abatement-Certification Letter").

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) A.D.D.J. Inc. at 9575 East 3rd Place, Denver, CO 80230: On or before August 20, 2011, the employer did not ensure that the path to ground was permanent and continuous for a splitter that was missing its ground plug, used to power a 110 volt Rol-Air air compressor, and two 110 volt portable circular saws. These tools were not double insulated, but were operated on a ground fault circuit interrupter (GFCI). Condition exposed employees to electrical shock hazards. Abatement certification is required for this item (see enclosed ,\"Sample Abatement-Certification Letter\").
