29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    Production Department - On or about July 29, 2011, the employer did not establish an energy control program for emloyees performing maintenance on the Hamblet Sheeter, Johnstone Rewinder and other operating equipment, thereby exposing employees to the hazard of unexpected energization of machinery.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points.    a.  Production Department - On or about July 29, 2011, machine guarding was not in place to protect employees exposed to in running nip points on the Cameron serial number P-409830 and Johnstone serial numbers 3375A rewinders, thereby exposing employees to amputation type injuries.     b.  Production Department - On or about July 29, 2011, the Clark Aiken Sheeter was not guarded to prevent employees from reaching into the in running nip points, thereby exposing employees to rotating parts.

29 CFR 1910.219(d)(1):     Guarding. Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punchpresses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.    Production Department - On or about July 29, 2011, the Johnstone Rewinder did not have guards affixed around the pulleys, thereby exposing employees to rotating parts.

29 CFR 1910.219(e)(1)(i):     Where both runs of horizontal belts are seven (7) feet or less from the floor level, the guard shall extend to at least fifteen (15) inches above the belt or to a standard height, except that where both runs of a horizontal belt are 42 inches or less from the floor, the belt shall be fully enclosed in accordance with paragraphs (m) and (o) of this section.    Production Departments - On or about July 29, 2011, the Johnstone Rewinder did not have a guard that fully enclosed the horizontal belts, thereby exposing employees to hazards associated with moving parts.

Where the drive extends over other machine or working areas, protection against falling shall be provided.This subparagraph does not apply to manually operated sprockets.    a.  Production Department - On or about July 29, 2011, a chain and sprocket on the Johnstone Rewinder was not completely guarded, thereby exposing employees to the hazards associated with moving parts.      b.  Production Department - On or about July 29, 2011, a chain and sprocket on the Cameron Rewinder was not completely guarded, thereby exposing employees to the hazards associated with moving parts.

Production Department - On or about July 29, 2011, the electrical wires running to the control box for the Johnstone Rewinder were not fully enclosed, thereby exposing employees to live electrical wires and electrocution type injuries.
