29 CFR 1910.303(b)(2): Listed or labeled equipment was not used or installed in accordance with instructions included in the listing or labeling:    a) On or about August 1, 2011, at the above located workplace employees working just east of the sorting area were exposed to an energized 120 volt floor fan that lacked proper grounding. Employees were thereby exposed to the hazards associated with electric shock.

29 CFR 1910.334(a)(3)(ii): Attachment plugs and receptacles were connected or altered in a manner which prevented proper continuity of the equipment grounding conductor at the point where plug(s) are attached to the receptacle(s):    a) On or about August 1, 2011, at the above located work place, employees working just east of the sorting area were exposed to an energized 120 volt floor fan which was missing a grounding pin. The employees using this equipment were thereby exposed to the hazards associated with electrical shock.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there was potential electrical hazards were not provided with, and/or did not use, electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed.    a) Between May 1, 2011 and August 1, 2011, at the above located work place, employees performing voltage testing on electrical equipment of 220 to 440 volts, were not provided and/or required to use personal protective equipment including but not limited to flame resistant clothing, leather gloves, voltage-rated gloves and double layer switching hoods or arc flash suit hoods when working on or near energized electrical (440 volt) parts. Employees were thereby exposed to hazards associated with energized electrical parts.
