29 CFR 1926.352(a): When practical, objects to be welded, cut, or heated shall be moved to a designated safe location or, if the objects to be welded, cut, or heated cannot be readily moved, all movable fire hazards in the vicinity shall be taken to a safe place, or otherwise protected. Lamar Construction Co. on or about August 22, 2011, at the worksite located at 2125 Wood Bridge Blvd. in Bowling Green, OH: The employer did not assure that all fire hazards were removed from areas where employees were conducting welding operations. Roofing insulation that was not removed from the vicinity before welding was conducted directly below the stored material exposed employees to fire hazards in that the insulation caught fire. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and verticalsurface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrailsystems, safety net systems, or personal fall arrest systems. Lamar Construction Co. on or about August 22, 2011, at the worksite located at 2125 Wood Bridge Blvd. in Bowling Green, OH: The employer did not assure employees were protected from falls when tasked to access the roof of a commercial structure to extinguish a fire. The roofs unprotected edges exposed employees to fall hazards of approximately 63 feet. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
