29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On August 22, 2011 at a worksite located at 4901 Forest Ave, Downers Grove IL. Employees were working on the roof of a residential house prepping for roofing work and did not have any means of fall protection. Employees were exposed to a fall of approximately 12.7 feet.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.1053(b)(4):     Ladder(s) were used for purposes other than the purpose for which they were designed:    On August 22, 2011 at a worksite located at 4901 Forest Ave, Downers Grove IL. An employee was using an A frame ladder as a straight ladder on a lower roof to access a higher roof.  The employee was exposed to a fall hazard.    Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    On August 22, 2011 at a worksite located at 4901 Forest Ave, Downers Grove IL. Employees were using a ladder to access the roof. The ladder was not extended three feet above the landing surface exposing employees to a fall hazard.     Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
