29 CFR 1910.24(e):     Fixed stairs were not installed at angles to the horizontal of between 30 deg. and 50 deg. A uniform combination of rise/tread dimensions were not used that would have resulted in a stairway at an angle to the horizontal within the permissible range.     A set of stairs leading up to the metal deck were not evenly sized in height and depth to produce a uniform angle of rise/tread dimensions.    Abatement certification required per 29 CFR 1903.19(c).

The sample was collected on October 11th, 2011 during a 442 minute sampling period; exposure calculations include a zero increment for the 38 minutes not sampled.    (b)  The grinder working on the heavy grinding floor was exposed to Respirable dust at an 8 hour time weighted average of 16.69 mg/m3, approximately 1.11 times the limit of 15 mg/m3; this limit is established to prevent irritation of the eyes, skin, respiratory system, cough; metal fume fever; siderosis; and iron staining of the eyes. 29 CFR 1910.1000(e):     Feasible administrative or engineering controls were not implemented to reduce employee exposure(s):    (a)  The grinder working on the heavy grinding floor was exposed to iron oxide dust at an 8 hour time weighted average of 15.65 milligrams per cubic meter (mg/m3), approximately 1.57 times the limit of 10 mg/m3; this limit is established to prevent irritation of the eyes, skin, respiratory system, cough; metal fume fever; siderosis; and iron staining of the eyes.  The sample was collected on October 11th, 2011 during a 442 minute sampling period; exposure calculations include a zero increment for the 38 minutes not sampled.    (b)  The grinder working on the heavy grinding floor was exposed to iron oxide dust at an 8 hour time weighted average of 16.69 mg/m3, approximately 1.67 times the limit of 10 mg/m3; this limit is established to prevent irritation of the eyes, skin, respiratory system, cough; metal fume fever; siderosis; and iron staining of the eyes.  The sample was collected on October 11th, 2011 during a 445 minute sampling period; exposure calculations include a zero increment for the 35 minutes not sampled.    Abatement normally will be multi-step as follows:    STEP 1:  Effective respiratory protection shall be provided and used for exposed employees as an interim protective measure until feasible engineering and/or administrative controls can be implemented, or whenever such controls fail to reduce employee exposure to within permissible exposure limits.    STEP 1 abatement date is 1 day:  ________________________________    STEP 2:  Submit to the Area Director a written, detailed plan of abatement outlining a schedule for the implementation of engineering and/or administrative measures to control employee exposure to hazardous substances as referenced in this citation.  This plan shall include, at a minimum, target dates for the following actions which must be consistant with the abatement dates required by this citation.    1.  Evaluation of engineering/administrative control options;    2.  Selection of optimum control methods and completion of design;    3.  Procurement, installation, and operation of selected control measures; and    4.  Testing and acceptable or midification/redesign of controls.    All proposed control measures shall be apporoved for each particular use by a competent industrial hygienist or other technically qualified person.    Thirty (30) day progress report is required during the abatement period.
