29 CFR 1910.23(c)(1):     Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:    On August 17, 2011, during cleaning activities, employees were walking beams approximately 8 feet above the stage roof to remove debris and were not protected from falls.

29 CFR 1910.23(c)(2):     Runways four feet or more above floor or ground level were not guarded by standard railings (or the equivalent) and toeboards:  Note:  Toeboards are only required where tools, machine parts, or materials are likely to be used on the runway.    1. On August 17, 2011, during cleaning activities, employees were using a runway to access a upper level and the runway was not properly guarded to prevent the employees from falling to ther floor below.    2. On August 17, 2011, during cleaning activities, the landing platform between the fixed ladder and the runway was not properly guarded to prevent the employees from falling to ther floor below.

29 CFR 1910.27(d)(3):     The employer did not ensure that the side rails of a through ladder extended 3 1/2 feet above parapets and landings, omitting the rungs from the extension and having not less than 18 nor more than 24 inches clearance between the rails.     On August 17, 2011, during cleaning activities, the fixed ladder leading to the platform to access the runway did not extend 3 1/2 feet above the landing to prevent the employees from falling to the floor below.
