29 CFR 1926.1053(b)(1): Portable ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, the the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder.      a) Property Construction, LLC - Brooklyn Center, Minnesota: On or about August 24, 2011, at address 6541 Still Waters Drive, Findlay, Ohio, the employer failed to ensure that employees were protected from falls while climbing on and off of a residential roof have a 10:12 pitch with a metal ladder that was not extended at least 3 feet above the upper surface. The ladder was located on the northwest side of the house to access the northwest peak. Employees were exposed to falls of approximately 10 feet to the ground below.     b) Property Construction, LLC - Brooklyn Center, Minnesota: On or about August 24, 2011, at address 6541 Still Waters Drive, Findlay, Ohio, the employer failed to ensure that employees were protected from falls while climbing on and off of a residential roof have a 10:12 pitch with a metal ladder that was not extended at least 3 feet above the upper surface. The ladder was located on the southeast side of the house to access the southeast peak. Employees were exposed to falls of approximately 10 feet to the ground below.     c) Property Construction, LLC - Brooklyn Center, Minnesota: On or about August 24, 2011, at address 6541 Still Waters Drive, Findlay, Ohio, the employer failed to ensure that employees were protected from falls while climbing on and off of a residential roof have a 10:12 pitch with a metal ladder that was not extended at least 3 feet above the upper surface. The ladder was located at the front of the house to access the southeast peak. Employees were exposed to falls of approximately 10 feet to the ground below.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet(1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system.    a) Property Construction, LLC - Brooklyn Center, Minnesota: On or about August 24, 2011, at address 6541 Still Waters Drive, Findlay, Ohio, the employer failed to ensure that employees were protected from falls while roofing on a 10:12 sloped roof on a residential type structure.  Employees were exposed to a fall of approximately 10 feet to the ground below.
