29 CFR 1926.501(b)(1): Each employee on a walking/working surface, with an unprotected side or edge which is 6 feet or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Mamrak's Masonry LLC - Toledo, Ohio: On or about August 23, 2011, at the address located at 896 Center Road, Avon, OH, the employer did not assure employees were protected from fall hazards while performing masonry work on top of a commercial structure. The employees were exposed to a 12 foot fall to the ground below.
