29 CFR 1910.23(c)(1):     Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall beguarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance toa ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides,    a) Railing leading into driveway basement was not equipped with a middle/intermediate railing.    b) Overhead platforn in the maintenance shop was not equipped with a standard guard railing system.    c) Employees were exposed to falling into the sump pit in that they crossed the pit on a twelve inch wooden board without no guardrails or equivalent.    d) There was no guardrail at the loading area in the feed mill to prevent employees from falls.

29 CFR 1910.27(d)(2)(i):     Where an employee had to step a distance greater than 12 inches from the centerline of the rung of a ladder to the nearest edge of structure or equipment, a landing platform shall be provided. The minimum step-across distance shall be 2 1/2 inches.    a) In the maintenance shop there was no platfrom to allow employees to step from the ladder to over head storage area.

29 CFR 1910.219(e)(3)(i):     Vertical and inclined belts shall be enclosed by a guard conforming to standards in paragraphs(m) and (o) of this section.    a) Belts on the bagging machines were not guarded to prevent employees exposure to pinch points.  b) Belt on the overhead conveyer was not guarded to prevent employees exposure to pinch points.

29 CFR 1910.219(c)(2)(i):     All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform,excepting runways used exclusively for oiling, or running adjustments, shall be protected by a stationary casing enclosing shafting completely or by atrough enclosing sides and top or sides and bottom of shafting as location requires.    a) Shaft attached to the Corn grinding machine was inadequate, in that the shield did not fully enclose the rotating part.  b) Shafts on the Mirco Batching machine was not guarded exposing employees to rotating parts.
