Instance 1.  On August 25, 2011 employees were exposed to the hazards associated with a cave-in while working in an unprotected trench at a depth of 5.9 feet.    Instance 2.  On August 25, 2011 employees were exposed to the hazards associated with a cave-in while working in an unprotected trench at depths between 6.2 and 7.2 feet deep.    Abatement documentation is required of this item in accordance with 29 CFR 1903.
