29 CFR 1910.23(c)(1) Every open-sided floor(s) and platform(s) 4 feet or more above adjacent floor or ground level was not guarded by a standard railing (or equivalent as specified in paragraph (e)(3) of this section) except where there is entrance to a ramp, stairway, or fixed ladder: On or about August 18, 2011, inadequate or missing fall protection was in place on the walkway by the crusher, in the middle of the process operation. The platform measured approximately 510 (5 feet 10 inches) above the ground level.
