29 CFR 1926.102(a)(1):  Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    a)  Aquilar, Guillermo at 14189 St. Paul Street, Denver, CO 80238:  On or before August 27, 2011, the employer did not ensure that employees wore safety glasses when using or working in the vicinity of pneumatic nail guns.  Condition exposed employees to the hazard of projectiles to the eyes. 29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet(1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems:    a)  Aquilar, Guillermo at 14189 St. Paul Street, Denver, CO 80238:  On or before August 27, 2011, the employer did not ensure that employees were using fall protection when framing this residence in the following instances:      1)  The employees were framing on the main floor and did not guard the basement stairway hole measuring 8' x 9' with a 10' fall hazard; and    2)  Three basement window wells on the south side, each measuring 3' x 4' with a 5'8" fall hazard, were not guarded.
