Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being caught or struck by conveyors:    On or about 8/22/11 at the base of the Standley Batch Systems, Inc. incline conveyor, employees were exposed to being caught or struck by conveyors due to (a) an audible warning device in conjunction with a visual warning light was not actuated by the controller device prior to starting the conveyor, (b) hazardous locations were not equipped with operational pull cord emergency stop devices, (c) personnel working near the conveyor were not instructed as to the location and operation of pertinent devices, (d) there were no routine inspections to ensure all guards and safety features properly functioned, and (e) warning signage was not provided at transfer areas.      Among other methods, one feasible and acceptable abatement method to correct this hazardous condition would be to implement safeguards as described in ASME B20.1 - 2009 Safety Standard for Conveyors and Related Equipment.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.23(d)(1)(ii):  Stairways having four or more risers that were less than 44 inches wide did not have a stair railing on an open side:    At the stairs leading to the underside of the holding container silos and collector/incline conveyors, the 80 inch high, 35 inch wide stairs consisting of nine risers, was not equipped with a stair railing on an open side.  The hazard exposed employees to fall hazards.    Corrected during inspection.

29 CFR 1910.24(f):  Stair treads were not slip resistant:    At the stairs leading to the underside of the holding container silos and collector/incline conveyors, the 80 inch high, 35 inch wide stairs consisting of nine risers, the block construction was damaged at the fifth riser from the bottom.  The hazard exposed employees to fall hazards    Corrected during inspection.
