OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that wire rope clips were installed incorrectly. (a) Employees were exposed to crushing hazards while working a the large and small foam press and the employer did not ensure that the Crosby wire clips used on the press were properly installed.

29 CFR 1910.212(a)(1) One or more methods of machine guarding shall be provided to protect the operatorand other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. (a) Employees were exposed to amputation hazards while operating the FMB Bandsaw (Model-Jupiter and Serial #CO1565) and the employer did not ensure that the unused portion of the blade was guarded. (b) Employees were exposed to amputation hazards while working on the Standard Industrial Corp. Shear (Model #AS 250-12 and Serial #1199879) and the employer did not ensure that employees were properly guard from the hold downs.

(b) Employees were exposed to amputation hazards while working with the Amada Press Brake (Model #HFE 130-4S and Serial #N010404) and the the employer did not ensure that employees were properly guarded from the point of operation.
