29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above a lower level were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:\r\n\r\na) On or about July 26, 2011 and times prior, employees were exposed to injuries from falls greater than 6 feet while installing roofing materials. The employee was wearing a full body harness attached to a rope, with enough slack in the rope that he could fall off the edge of the roof.

29 CFR 1926.503(c)(3): When inadequacies in an affected employee's knowledge and use of fall protection indicated that he did not have the understanding or skill required by paragraph (a) of this section, the employer did not retrain his employee: a) On or about July 26, 2011, and at times prior, an employee wore a full body harness and a rope that extended well beyond the edge of the roof. His fall protection system did not include a shock absorber or a rope grab.
