29 CFR 1926.100(a):     Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:      On August 31, 2011 an employee was not protected by a hard hat while working in a trench, exposing him to a struck by hazard.      Abatement certification is required of this item in accordance with 29 CFR 1903.

29 CFR 1926.651(i)(3):     Sidewalks, pavements, or appurtenant structures had been undermined and a support system or another method was not provided to protect employees from the possible collapse of such structures:      On August 31, 2011 an employee was allowed to work under an undercut sidewalk, exposing him to a struck by hazard.    Abatement certification is required of this item in accordance with 29 CFR 1903.
