29 CFR 1910.132(a): Protective equipment was not used when necessary and was not maintained in a reliable condition whenever hazards capable of causing injury and impairment were encountered: Landmark Services Cooperative, 6631 County Road M, Evansville, Wisconsin: On or about August 22, 2011, at the worksite located at 6524 County Road M, Evansville, Wisconsin, the employer did not provide fall protection equipment for employees who walked along the tops of rail cars prepping them for the loading of grain. Employees were exposed to a fall of 14.5 feet onto gravel and/or earthen materials.
