29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed: T-500 building - The employer failed to adequately conduct periodic inspections to ensure that energy control procedures for conveyor equipment complied with this standard. Employees performing decontamination of conveyer equipment were exposed to caught between and amputation hazards. The employer's energy control program procedures did not comply with 29 CFR 1910.147(c)(3) and no lockout was provided for employee protection.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): T-500 Building - Employees performing decontamination of conveyer equipment were exposed to caught between and amputation hazards and the employer did not perform one or more of the procedures for the application of energy control in the sequence outlined in 29 CFR 1910.147(d)(1)-(6).
